Wellness Provider Application

This position is remote. All sessions will take place online.


  • Embody the company standard as a high-quality care provider.

  • Complete anti-racist training.

  • Ability to complete all documentation in a timely and comprehensive manner, including but not limited to state-required documentation, treatment plans, assessments, progress notes, and discharge summaries.

  • Identify and address the psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families in a friendly and sympathetic manner.

  • Coordinate the referral of patients and families to appropriate community, social service, other extended care, and rehabilitation facilities and organizations.

  • Submit all required documents in a timely manner. Maintain confidentiality.

  • Ensure a safe and comfortable environment.


The Grace Program will work with you as you build a caseload that is manageable. We recognize that it may take time to build a solid caseload and are willing to work with you on a part-time basis until you become full-time, if that is your desire. We are also seeking qualified candidates who may want a part-time job to supplement their income.


  • Certified life coaches and fitness trainers or

  • Licensed therapists and mental health workers.

We accept applications on a rolling basis. You must attend our onboarding training before you can begin.
